Upon requesting registration you will be asked to sign the Risks & Liability Waiver form for all participants you are signing up.
1) Voluntary Participation & Fitness to Participate
I, the undersigned participant (or parent/guardian of a minor), understand participation in activities offered by All-Star Flag Football Training LLC is voluntary. I affirm the participant is physically fit and able to participate and have no medical condition that would make participation unsafe. I agree to disclose any relevant medical conditions or limitations to staff before participating. By signing this waiver, I am affirming that I give permission for the participant to partake in various physical activities and confirm that I have read and understand the risks and terms and conditions in this waiver agreement.
2) Assumption of Inherent Risks
I understand that sports, fitness, training, and related activities involve inherent and obvious risks that cannot be eliminated without jeopardizing the essential qualities of the activity. These risks include, but are not limited to:
- Slips, trips, falls, collisions with objects or persons; which can cause abrasions, bruises, cuts, turf/grass burn or joint injuries;
- Overexertion, heat exhaustion, dehydration, muscle strains/sprains, fractures, tendon injuries; such as fatigue, dizziness, or physical stress,
- Equipment malfunction or misuse; facility hazards; environmental and field conditions; such holes, wet patches, debris, and other surface hazards leading to ankle rolls or falls
- Actions, inactions, or negligence of other participants;
2.A) Concussion Waiver
As Flag Football is designed to be a minimal contact sport, there is still a low risk for concussion. As part of this acknowledgement, we ask you to read and by signing you understand the following:
- Concussion, is a serious type of traumatic brain injury (TBI) caused by impact from an object, person or fall to the head. Whether it be a bump, blow, jolt, or hit, this impact causes the head and brain to move rapidly back and forth, causing the brain to bounce around or twist in the skull, creating chemical changes in the brain and sometimes stretching and damaging the brain cells.

I knowingly and voluntarily assume all such risks, both known and unknown, including risks arising from the ordinary negligence of All-Star Flag Football Training LLC, its owners, officers, employees, contractors, and agents (collectively, the “Released Parties”), to the fullest extent permitted by law.
3) Release of Liability & Waiver of Claims
To the maximum extent permitted by applicable law, I release, waive, discharge, and covenant not to sue the Released Parties from any and all claims, demands, causes of action, damages, or liabilities arising out of or related to my participation, including claims arising from the ordinary negligence of the Released Parties.
This release does not apply to gross negligence, willful or wanton misconduct, or where otherwise prohibited by law.
4) Indemnification & Hold Harmless
I agree to defend, indemnify, and hold harmless the Released Parties from any claims, demands, damages, costs, and attorney’s fees arising from my participation or my breach of this Agreement, including claims by third parties or other participants to the extent allowed by law.
5) Safety Rules & Compliance
I agree to follow all posted and verbal rules, instructions, and safety protocols, use provided protective equipment as directed, and cease participation if I observe any unsafe condition. I understand staff may remove participants for unsafe behavior without refund.
6) Medical Authorization & Insurance
In the event of injury or medical emergency, I authorize the Released Parties to secure first aid and emergency medical care as deemed necessary. I authorize, give permission and voluntarily consent to All-Star Flag Football Training LLC to arrange, direct, sign for and consent to all routine or emergency medical care and treatment necessary to preserve my child’s health in the event of accident, injury, sickness, etc. during participation in activities offered by All-Star Flag Football Training LLC. I understand I am responsible for all associated costs and I am encouraged to maintain personal health insurance. I further acknowledge that All-Star Flag Football Training LLC have not made any guarantees as to the effect of such care and treatment rendered.
7) Personal Property
I understand All-Star Flag Football Training, LLC is not responsible for lost, stolen, or damaged personal items brought onto the premises.
8) Minors (if participant is under 18)
I am the parent/legal guardian of the minor participant named above. I have read and understand this Agreement, and I consent to the minor’s participation. I agree to all terms herein on the minor’s behalf, including the release, waiver, assumption of risk, and indemnification provisions, to the fullest extent permitted by law.

9) Cancellations, Refunds & Scheduling
I acknowledge the business’s cancellation and refund policy for classes, sessions, and events:
Cancellation Notice:
Participants may receive a full refund if they provide written notice by email or text at least 24 hours prior to the scheduled start time of the event, class, or training session they registered for.
Missed Sessions or Partial Participation:
If a participant misses any portion of an event, including late arrival, early departure, or absence from individual sessions within a multi‑day program, no refund will be issued.
However, to support continued skill development, any participant who misses part or all of an event will be allowed to attend a future comparable event at no additional cost, subject to availability.
No‑Show Policy:
Participants who do not provide at least 24 hours’ notice and do not attend the event are not eligible for a refund but may still redeem a future event credit under the policy above.
Transferability:
Credits for future events may only be used by the original participant and are not transferable to other individuals unless approved by the business.
10) Arbitration / Governing Law / Venue
Any dispute arising from this Agreement shall be resolved by binding arbitration administered by [Arbitration Body or “a mutually agreed arbitrator”] under its rules. Judgment on the award may be entered in any court of competent jurisdiction. This Agreement shall be governed by the laws of the State of New Mexico, and venue shall lie in Sandoval County New Mexico.
If local law restricts arbitration clauses or requires specific consumer disclosures, consult counsel to modify this section.
11) Severability & Entire Agreement
If any provision of this Agreement is found unenforceable, the remaining provisions shall remain in full force. This Agreement constitutes the entire understanding regarding the subject matter and supersedes prior discussions.
12) Photo, Video & Media Release
I grant All-Star Flag Football Training, LLC the right to photograph, film, and/or record my participation and to use my name, image, likeness, and voice in promotional materials, online media, and publications without compensation.

13) Acknowledgment & Signature
By signing below, I represent that I have carefully read this Agreement, understand its terms, and sign it voluntarily. I understand that I am giving up substantial legal rights by signing, including the right to sue for ordinary negligence, to the extent permitted by law.
